High-resolution manometry and impedance-pH/manometry: novel techniques for the advancement of knowledge on esophageal function and their clinical role.

چکیده
The study of esophageal function is based on the study of esophageal motor activity using conventional esophageal manometry (CEM), which has proven useful in the diagnosis of patients with functional dysphagia and other esophageal motor disorders (1). Most consensus guidelines recommend contractile activity monitorization with 4 to 8 sensors, including a 6-cm “e-sleeve” to minimize displacement at the lower esophageal sphincter (LES) (2). In the last few years new techniques are allowing a better understanding of esophageal motor disorders. These include high-resolution manometry (HRM) and impedance combined with manometry. Both are relevant research tools that are being increasingly implemented in daily clinical practice (3). The study of pathological acid reflux as a potential cause for most motor disorders and symptoms as experienced by patients with functional esophageal disease is undertaken with a 24-hour ambulatory pH-metry with one or several recording sites, and is still considered the gold standard for the study of non-erosive gastroesophageal reflux disease (GERD). However, there is a growing interest in the diagnosis of patients with atypical reflux complaints and of those refractory to therapy with proton-pump inhibitors (PPIs), where pH-metry plus impedance plays a relevant role given its ability to detect weakly acidic or non-acidic reflux episodes (4).

Relationship between esophageal clinical symptoms and manometry findings in patients with esophageal motility disorders: a cross-sectional study
Background: Manometry is the gold-standard diagnostic test for motility disorders in the esophagus. The development of high-resolution manometry catheters and software displays of manometry recordings in color-coded pressure plots have changed the diagnostic assessment of esophageal disease. The diagnostic value of particular esophageal clinical symptoms among patients suspected of esophageal m...
متن کاملSleeve sensor versus high-resolution manometry for the detection of transient lower esophageal sphincter relaxations.
Transient lower esophageal sphincter relaxations (TLESRs) are the most important mechanism by which gastroesophageal reflux occurs, and sleeve sensor manometry is the gold standard for detection of TLESRs. The aim of this study was to evaluate manometry with closely spaced sideholes (high-resolution manometry) for the detection of TLESRs as an alternative. In 12 patients with gastroesophageal r...

متن کاملAchalasia Previously Diagnosed as Gastroesophageal Reflux Disease by Relying on Esophageal Impedance-pH Monitoring: Use of High-Resolution Esophageal Manometry in Children
Gastroesophageal reflux disorder (GERD) is the most common esophageal disorder in children. Achalasia occurs less commonly but has similar symptoms to GERD. A nine-year old boy presented with vomiting, heartburn, and nocturnal cough. The esophageal impedance-pH monitor revealed nonacidic GERD (all-refluxate clearance percent time of 20.9%). His symptoms persisted despite medical treatment for G...
متن کاملClinical Applications of Esophageal Impedance Monitoring and High-Resolution Manometry
Esophageal impedance monitoring and high-resolution manometry (HRM) are useful tools in the diagnostic work-up of patients with upper gastrointestinal complaints. Impedance monitoring increases the diagnostic yield for gastroesophageal reflux disease in adults and children and has become the gold standard in the diagnostic work-up of reflux symptoms. Its role in the work-up for belching disorde...
